It's always a little awkward and disturbing to hear the toilet flush after you've been on the phone with them for a few minutes. Sending a loud and clear AMEN!
Another awkward moment: when you're in a public restroom and the person next to you is carrying on a phone conservation in their stall. Do you flush repeatedly to rat the person out? Hmm..
Once I heard a lady in the stall next to me chatting away while her vigorous stream of urine was zinging out a happy tune against the porcelain bowl and I thought, "Doesn't she know that her fellow converser HAS to be hearing that???" Then there was the lady on her phone who was standing in a long line at the grocery store and loudly (and vividly) describing her present YEAST INFECTION. Wow, compared to some people, I feel positively CLASSY. (Imagine that!)

YES! I agree completely! I don't want to hear ANY sort of running water in the backgound when I'm on the phone with someone.

It's not a deal breaker for me, but I always remind them to wash their hands..:)
